WAAG Starter's Course Application Form is in Adobe Acrobat format. Download and print the application form, complete the details and send to the address included on the form.

ADAA Membership Form is in Adobe Acrobat format. Download and print form then complete the details and send to the ADAA secretary whose address is on the form. Don't forget to also fill in the dog registration form for each dog you are registering.

ADAA Dog Registration Form is in Adobe Acrobat format. Download and print form then complete the details and send to the ADAA secretary whose address is on the form. Upon joining ADAA there is no cost for dog registration. If you an existing member and have a new dog then the cost is $5 per dog.

ADAA Competition Entry Forms. Download and print then complete the details for each competition. (We often photocopy partially completed forms so you only need to complete the event details for each competition).

ADAA Title Application Forms. Congratulations you have acheived a title! Download and Print this form and send in with the required fee. Don't forget to include your qualification cards (they will be sent back with your title).
